Job summary

The post holder will work with the operations manager, clinical lead and other senior nurses to understand the clinical service requirements and help implement plans for service improvement and involve patients to ensure changes to services meet patients expectations as well as ensuring the required capacity to meet key clinical performance targets. They will work as part of the Urgent Treatment Centre team. The post holder will be responsible for providing high quality health care and information to the public. An opportunity exists for an experienced and highly motivated nurse with relevant experience who is committed to making a valuable contribution to the further development of these successful and pioneering services.

The services offer treatment for minor injuries and minor ailments 365 days per year, and are open from 8am 9 pm.

If you have post qualification experience in A&E, Walk-in Centre, or Urgent Care with recent experience in the treatment of minor ailments and injuries and significant management experience.

Main duties of the job

The successful candidate will perform holistic assessments of patient nursing needs. Plan, assess, implement and evaluate care delivery according to changing health needs utilising prescribing skills.

Ensure practice is evidence based through undertaking audit and critically appraising literature to achieve a high standard of care.

The post holder actively supports the District Nursing team by undertaking responsibility for a delegated caseload.

Liaise with the District Nurse team leader to ensure information sharing in relation to patient care / changes to care plan and the continuity of care and reviewing of complex care packages.

Mentoring, Teaching and Assessing.

Liaises with all health care professionals and other agencies to ensure effective service delivery.

Involve patients and carers in the planning and delivery of care.

Maintain accurate records in accordance with NMC Standards.

To participate in the health needs assessment of the practice population to identify areas of unmet need.

Ability to work without direct supervision and to solve problems.

Work on a rota basis to facilitate weekend working across the district nursing teams

Duties and Responsibilities

Undertake a range of clinical procedures, which may include, suturing, venepuncture, cannulation, ear syringing, slit lamp use, removal of foreign bodies.

Participate in and oversee triage to ensure smooth progress of patients through the system.

Identify at risk patients and manage appropriately in accordance with national and local policy and procedures i.e. vulnerable children, adults and child protection.

Collaborate with the identified Manager in the co-ordination of services and patients within the urgent care centre.

Act as an advocate for the patient at all times.

Managerial

To undertake role of clinical leader and lead the team to fulfil the operational requirements and service delivery of the service.
