Job responsibilities
To act as a senior clinical
professional within the urgent treatment centre; liaising with the team and
colleagues across the wider social and health care environment to promote,
maintain and enhance effective multidisciplinary patient care and maintain a
high quality service to patients attending the Urgent Treatment Centre
To act as an expert
independent autonomous practitioner; providing specialist advice to patients
and staff contributing to assessment, diagnosis, treatment and care of clients
presenting with undiagnosed and undifferentiated health care problems.
Possess
and demonstrate extended skills and experience in the management of a wide
range of health care problems i.e. minor illness and injury. Exercise
professional autonomy and clinical judgement in managing or referring clients
to a variety of primary and secondary care services as appropriate.
Duties
and Responsibilities
To
undertake the assessment, planning, implementation and evaluation of care for
patients presenting with an undifferentiated diagnosis in a primary care
setting and formulate, management plans following history taking and clinical
examination, autonomously within legislative parameters.
To provide expert critical and analytical
decision-making skills which have been assessed at a higher level. Possessing
proven skills in the management of a
wide range of health care problems i.e. minor illness and injury, wound care,
chronic disease management.
To promote health, both systematically and
opportunistically to effectively manage acute and chronic health problems and
provide health promotion education
To practice at an expert level demonstrating in
depth knowledge and competence in all aspects of first contact and access,
encompassing management of minor illnesses, trauma and mental health which is
evidence based to provide effectively managed care for individual care.
To improve outcomes in patient satisfaction,
health status, costs and process of care, and to present alternative treatment
choices to patients
To refer patients to other agencies, e.g.,
secondary care, independently,
as and when appropriate and to refer patients for further investigations as
required.
Initiate and conduct clinical investigations as
appropriate
To interpret results of clinical
investigations, and act as required, including the communication of such
results to patients.
Supply
and administer medications as appropriate via PGD
To understand the legislation relevant to the
practice of nurse prescribing. To understand the roles and relationships of
others involved in prescribing, supplying and administering medicines.
Exercise
independent and delegated prescribing rights in accordance with current and
legal and Trust criteria, and within the clinical situation.
To
maintain an accurate patient electronic record using the clinical information
system.
To
provide expert opinion to all other members of the Urgent Care Centre.
Undertake
a range of clinical procedures, which may include, suturing, venepuncture,
cannulation, ear syringing, slit lamp use, removal of foreign bodies.
Participate
in and oversee triage to ensure smooth progress of patients through the system.
Identify
at risk patients and manage appropriately in accordance with national and local
policy and procedures i.e. vulnerable children, adults and child protection.
Collaborate
with the identified Manager in the co-ordination of services and patients
within the urgent care centre.
Act as
an advocate for the patient at all times.
Managerial
To undertake role of clinical leader and lead
the team to fulfil the operational requirements and service delivery of the
service.
To deputise for the clinical lead/operational
manager in their absence.
Supervise
and maintain high standards of professional care in accordance with current NMC
Code of Professional Conduct, Scope of Professional Practice and ethical
considerations.
Co-ordinate
the work flow for the shift and ensure appropriate hand over to the following
shift to enable seamless effective care to be provided to service users.
Provide
a supportive environment for staff, take responsibility for and perform in
clinical supervision and personal development planning.
Assist
the Clinical Lead in shaping the service, to be welcoming and effective to the
needs of service users.
Participate
in the development of an effective nurse-led service utilizing a holistic,
health needs approach.
Supervision
of the supply of medicines by junior staff as appropriate via PGD.
Manage
and co-ordinate medicine stock control
To
formulate and implement policy and guidelines to improve access, to reduce
inequalities and to support new role development and new ways of working.
Develop
operational policies and procedures at practice level for use within the Urgent
Treatment Centre.
Authorised
signatory for stores and equipment.
Leadership
Fulfil
the role of clinical leader on a day-to day basis.
Delegate
responsibility where appropriate and encourage a culture where flexibility and
adaptability are positively encouraged.
Support
and maintain the focus of the service.
Adopt
and promote a positive approach to change.
Actively
participate in and promote the development of professional nursing practice.
Manage
team dynamics
Lead
the development of a service which is welcoming, effective and responsive to
the needs of service users
Develop
clear systems of communication and referral both within the UTC and across
services in primary and secondary care.
Provide
expert nursing advice, and support to practitioners and team members on a
regular basis and respond to requests for advice which may arise from both
within and outside the services.
Responsible
for the delivery of evidence- based effective clinical care to clients
attending service.
Responsible
for setting staff objectives and maintaining a system of review.
Research and Audit
Initiate and actively participate in audit and
evaluation of the service e.g. A&E feedback, demographic service needs,
laboratory results to promote change to practice.
To continually assess current practice through
the use of planning and evaluation of clinical audit
To
participate in research trials and undertake systematic reviews.
Promote a research
culture within the Urgent Treatment Centre, and actively promote the use,
publication and dissemination of research evidence.
To critically appraise both primary and
secondary research and translate these findings to clinical practice.
To implement recommendations of recognised
central bodies, e.g.. The National Institute for Clinical Excellence, through
liaison with the Primary Health Care Team, and other stakeholders.
To develop benchmarks of best practice.
Training and Education
To
share knowledge and skills with patient and their families as a key therapeutic
intervention and to enable self-care and autonomy.
To
teach and give an insight into the knowledge and skills framework required by
nurse clinicians working at an advanced level. This framework may consist of
patho-physiology, pharmacology, anatomy, physiology, assessing health needs and
clinical examination skills; providing
education and training.
Actively participate and lead in the identification
of education and training needs of the team; being responsible for identification
of educational and training needs of staff across the service.
Demonstrate a positive commitment to the training
and education of all staff within the interdependent teams and those people
whom interface with the service as required.
Maintain and develop
own clinical and professional expertise by participation in Clinical
Supervision and Personal Development Planning, in accordance with Nursing and
Midwifery Council Code of conduct, and scope of professional Practice.
Provide supervision
and mentoring opportunities for Urgent Treatment Centre team members and
students.
Participate in formal
and informal teaching sessions to promote clinical excellence within the Urgent
Treatment Centre team.
Provide
mentorship and placements for a range of pre-and post-registration nursing
students, masters level students and specialist practitioner students.
Communication
Establish and
maintain collaborative working with colleagues in primary, secondary and social
care.
Maintain and promote
effective communication and working relationships with colleagues across the
local health economy.
Maintain effective
systems of communication and referral both within the Urgent Care centre and
across primary and secondary care.
Promote and maintain
effective comprehensive clinical documentation in accordance with local and NMC
guidelines for record keeping.
Maintain
confidentiality at all times in accordance with local and National
requirements.
Demonstrate
professional skills in facilitating effective and clear communication in
dealing with sensitive or volatile situations/issues i.e. child protection,
alcohol/drug abuse, sexual orientation, racial and ethnic diversity, mental
illness, domestic violence, physical deformity and others.
Possess and
demonstrate cultural competence by interacting effectively with clients from
many backgrounds.
Utilise the relevant
Trust e-mail communication system.
Utilise a variety of
systems to communicate clinical information in accordance with the requirements
of the data protection act (fax, telephone, letter and email)
