Job summary
Woodsley and Holt Park PCN is looking for an enthusiastic part time
pharmacist (3 days a week) to join the well established team of 9 pharmacists and 4 pharmacy technicians. This role will
give you the opportunity to develop your clinical skills in a patient-facing
environment.
The PCN is made up of ten practices covering a diverse population in
Leeds. You will work closely with 2 of the
practices and have the benefit of support from General Practice staff, the
existing pharmacy team and links into the wider MDT employed by the PCN.
The PCN aims to provide the
best patient care possible. You will work in a patient-facing role, supporting
patients to get the most benefit from their medicines through Structured
Medication Reviews.
You will also provide support
to the wider MDT with
- medication and prescription queries
- acute requests for medication
- medicines reconciliation following transfer of care
- QOF/IIF/local incentives
- prescribing processes
- medicines policies and procedures
- audit and medicines safety
You will be a registered pharmacist and have experience of working as a pharmacist in a community, primary care or hospital setting.
You will be committed to providing outstanding patient care, have exceptional communication skills and you will be a good team player while being capable of independent and autonomous work.
As part of this role you will complete the CPPE Primary Care Pharmacy Education Pathway and undertake Independent Prescribing training, if not already completed.
Main duties of the job
To support
Woodsley and Holt Park PCN with all aspects of medicines optimisation.
The post holder
will work within their clinical competencies as part of a multi-disciplinary
team to provide expertise in clinical medicines optimisation across the PCN.
This will include structured medication reviews with direct patient contact and
may include contributing to:
- Management of
long-term conditions
- Minor illness
- Management of
medicines on transfer of care
- Reviewing
systems for safer prescribing
- Contributing to
repeat prescription authorisations and reauthorisation
- Actioning acute
prescription requests
- Addressing both
the public health and social care needs of patients
- Contributing to
achievement of IIF, QOF and locally commissioned quality improvement schemes
- Undertaking
clinical audit
The specifics
of the role in each PCN will be worked up between the post-holder, the PCN and
the allocated surgeries.
