We have an exciting opportunity for motivated diabetes facilitator who is enthusiastic about diabetes to join our supportive and dynamic clinical team.

We have an opportunity for:

  • Two band 4 diabetes facilitators to join the diabetes service
  • We have the opportunity for one fulltime and one part time post
  • These roles work across both areas of the diabetes service. They will work within the Leeds programme delivering group based education in a virtual and face to face setting for those newly diagnosed with type 2 diabetes. The post holder will also work as part of the MDT diabetes service supporting patients with complex type 2 diabetes.

The post includes some evening and weekend working.

Main duties of the job

The LEEDS Programme is a structured education 3 week course/full day session delivered to patients with Type 2 diabetes and forms part of the service delivered by the Community Diabetes Team. The sessions are delivered both virtual and face to face. This includes promotion of the service and educating patients/the public on services available to support their diabetes. Some sessions are delivered on a 1:1 basis but we also offer group support.

As a diabetes facilitator within the diabetes MDT service you will often be a first port of call for patients entering our service. You will support patients on their self-management journey and support the team in collecting blood glucose levels, completing general health checks and teaching patients to use newer technologies for supporting self-management. The role involves checking BP, blood glucose levels and some foot checks, as well telephone assessments, face to face assessments and home visits.

Your role will involve working across both areas of our service. We are looking for an excellent diabetes facilitator to work alongside other members of the team. It’s our priority to continue delivering the high quality diabetes care for which our service is known. You will have the opportunity to work alongside experienced staff who can help you develop your knowledge, skills and experience.
