Job summary
Assessment and Referral Clinician 0.5 / AHP (Occupational therapist / Physiotherapist) 0.5.
Within this role there is the need to think critically, and problem solve issues which may arise as part of the process of reviewing referrals and completing assessments. You will also be able to assist patients in recognising and identifying their own rehabilitation goals. The post holder will be involved in on-going service development specific to this role and prioritise and plan your own workload. This role will include a mixture of both working from home, home visits and office working.
We work closely with other members of the MDT to provide time limited, goal focused and patient-centred care. The role also involves working closely with other services such as the Neighbourhood Teams, housing, education, leisure, voluntary services and Adult Social Care. The post holder should be able to use effective communication skills and make clinical decisions.
Main duties of the job
The successful candidate will be dynamic, proactive and committed to improving patient care from the point of referral to discharge. This role requires the ability to use sound clinical judgment and high levels of communication and organisational skills and the ability to work to timescales. The post holder must also have relevant clinical experience in Neurology and have experience working within a multi-disciplinary team. The service works in patient’s homes, clinics and in the wider community settings such as care homes and leisure centres.

The candidate will have access to regular clinical supervision, and peer support from highly skilled clinicians both within the team but also the wider neurological service.
