Job summary

The Community Discharge Assessment Team (CDAT) aims to prevent avoidable hospital admissions and works closely with the multi-disciplinary team, providing expertise in discharge planning and ensuring a seamless transition between hospital and community services. CDAT Discharge Facilitators are a mixture of Registered Nurses, Physiotherapists and Occupational Therapists. The service aims to promote a good discharge experience for patients and their families/friends. Our main areas of work are the ED/SDEC (Same Day Emergency Care) and the acute assessment wards while working in collaboration with our community services. CDAT holistically assess patients and identify opportunities for timely discharge where the patient can be safely managed in the community and avoid unnecessary admission to hospital. The team works together with community and hospital colleagues, Adult Social Care and the third sector to deliver best outcomes for people.

Main duties of the job

Please note, it is a requirement of the role to be a registered Nurse, Physiotherapist or Occupational Therapist and you are required to maintain and update your registration in line with the Trusts Professional Registration Body. Discharge Facilitators use their skills to assess patients needs and work in collaboration with our community colleagues to achieve positive outcomes for the patient. The service is constantly striving to enhance its offer to improve patient experience. The service operates a 7 days service, 365 days a year 08:00 20:00hrs Mon-Friday and 08:00 18:00 Sat Sun and Bank Holidays. Each morning the team has a huddle to allocate wards, ED and SDEC (Emergency Department / Same Day Emergency Care). The team will then establish which patients need to be assessed. They will then visit each patient and co-ordinate their discharge. CDAT is commissioned to assess patients who are 16yrs and over.

The service is flexible and there are great opportunities for both nurses and AHPs within the Community Discharge Assessment Team while we go through a period of change. The Discharge Facilitators have excellent links into Leeds Teaching Hospitals Trust (LTHT), Community Teams, Adult Social Care and the Third Sector. Being part of Patient Flow enables you to be a part of a patients journey and contribute to their transfers of care from hospital and support them with the right outcome at the right time. The Community Discharge Assessment Team has recently started an Occupational Therapy pilot, this pilot means where available one OT will work out of Discharge Facilitator role and be available to support the remaining Discharge Facilitators with OT orientated tasks. The OT is available to carry out home/environmental visits and to start embedding the pull model at the front door to reduce the number of admissions into LTHT.

1. Clinical

1.1. Demonstrates a detailed theoretical and practical knowledge of common aetiologies, pathologies, conditions and presentations involving a critical understanding of theories and principles

1.2. Demonstrates innovative and specialist skills to select methods and tools to holistically assess, plan, implement and evaluate in a complex and specialised field of work.

1.3. Demonstrates highly developed physical skills where dexterity and accuracy are important e.g. syringe pumps and infusions, manual treatments

1.4. Makes consistent judgements requiring complex fact analysis, interpretation and comparison of options and ensures clinical reasoning is reflected accurately in case records

1.5. Engages and actively involves the individual and, if appropriate, their family/carer, in the assessment, planning, implementation and evaluation of programmes of treatment and/or care by using person-centred techniques to promote a culture of involvement and empowerment

1.6. Works to standards of proficiency identified by their professional and regulatory bodies performing clinical treatments/procedures to a competent standard

1.7. Prioritises their own workload, and that of the wider team, within agreed objectives deciding when to refer to others as appropriate.

1.8. Provides and receives complex, sensitive or contentious information where motivational, persuasive, empathetic, negotiating and reassurance skills are required whilst demonstrating an understanding of barriers to communication

1.9. Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all areas of practice including the management of any complaints and compliments.

1.10. To comply with the organisations Infection Prevention and Control requirements, including bare below the elbows dress code for staff with a direct care delivery role
