Job summary

An exciting new opportunity has arisen to work as a specialist diabetes pharmacist, in an innovative and forward thinking PCN.

Woodsley and Holt Park PCN are committed to developing a multidisciplinary Diabetes Support Team (DiaST) to support with the care of people living with diabetes and those who have been newly diagnosed. We are looking for a pharmacist who has specialist experience in diabetes to join a team of dieticians, health and well-being coaches and a podiatrist, to improve the care of people mainly (but not exclusively) with type 2 diabetes. This exciting new role will give you the opportunity to develop your specialist clinical skills in a patient-facing, primary care environment, working with a wide variety of healthcare professionals. You will be able to access support from senior clinical pharmacists specialising in diabetes across the city of Leeds and the consultant pharmacist for diabetes based at the West Yorkshire Integrated Care Board.

The PCN is made up of ten practices covering a diverse population in Leeds. You will provide your own clinics across the geographical area within GP surgeries and hub sites.

You will have the benefit of support from General Practice staff, the existing pharmacy team and links into the wider MDT employed by the PCN.

Main duties of the job

To work as a clinical diabetes specialist as part of the multidisciplinary specialist diabetes team.

To work as an autonomous practitioner, co-ordinating your own caseload to deliver an expert clinical service within diabetes

To provide specialist clinical advice to people living with diabetes, their carers, and other clinical staff to provide safe, effective and evidence based diabetes care.

To ensure that people with diabetes are fully informed and supported when making decisions about treatment.

You will be a registered pharmacist and have experience of working with people with diabetes.

You will be committed to providing outstanding patient care.

You will have exceptional communication skills.

You will be good team player while being capable of independent and autonomous work.

As part of this role you will complete the CPPE Primary Care Pharmacy Education Pathway and undertake Independent Prescribing training, if not already completed.

1. Clinical

1.1. Develops, leads and coordinates medicines optimisation services and a clinical pharmacy service for people living with diabetes within the PCN.

1.2. Enhances the existing general practice/PCN diabetes team, by bringing a different skill set to the table, to work collectively as part of the multi-disciplinary team to improve the care of people living with diabetes

1.3. Manages own clinical case load of people living with diabetes (either at practice or PCN level), utilising prescribing qualifications to improve health outcomes, reduce complications and reduce diabetes related hospital admissions.

1.4. Provides clinical leadership to ensure high quality and cost-effective prescribing in diabetes, and other therapeutic areas

1.5. Be an active member of and continue to build on the PCN DiAST team

1.6. Leads on the implementation of new drugs and technologies for diabetes.

1.7 Provides highly specialised pharmaceutical advice in complex situations (where advice may be challenged) to health care professionals, patients and carers.

1.8 Utilises advanced specialist knowledge covering a range of procedures and underpinned by relevant broad-based knowledge, experience and competence.

1.9 Develops and maintains new skills in extended scope practice in response to emerging knowledge and techniques.

1.10 Where appropriate to service delivery, demonstrates highly developed physical skills which require specific training or considerable experience to attain the level required e.g. advanced moving and handling techniques, advanced clinical assessment, clinical procedures such as foot checks or diagnostic tests.

1.11 Demonstrates independent practice responding to social, scientific, clinical and ethical issues which are encountered.

1.12 Makes complex judgements requiring analysis, interpretation and comparison of options by integrating complex knowledge sources in new and/or unfamiliar contexts and determining the appropriate application of clinical guidelines relevant to patient needs.

1.13 Responsible for developing new and innovative clinical pathways across the broad health and social care context

1.14 Works to standards of advanced proficiency identified by their professional and regulatory bodies performing clinical treatments/procedures to a highly specialist standard

1.15 Prioritises their own workload within agreed objectives deciding when to refer to others as appropriate.

1.16 Engages and actively involves the individual and, if appropriate, their family/carer, in the assessment, planning, implementation and evaluation of programmes of treatment and/or care by using person-centred techniques to promote a culture of self care, involvement and empowerment

1.18 Acts as an advocate for patients and their families/carers recognising the boundaries of their clinical knowledge; liaising and referring on to other services / agencies as required.

Essential

  • Post registration experience of working as a pharmacist caring for patients with diabetes
  • Experience of MDT working
  • Experience of collaborative working and of building relationships across a variety of organisations and the ability to integrate general practice with community and hospital pharmacy teams and community groups

Knowledge

  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing. Analyse and interpret complex prescribing information and present information in a clear way
  • Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based diabetes care.
  • Able to plan, manage, monitor and review general medicine optimisation issues in core areas for long term conditions

  • Mandatory registration with the General Pharmaceutical Council.
  • Professional knowledge acquired through vocational masters degree in pharmacy
