Job summary
We are seeking to appoint a Community Staff Nurse to work as part of our nursing team to ensure that we continue to deliver outstanding patient care within the patients home overnight. You will also support our team of Nursing Assistants to ensure seamless delivery of high quality patient care.
The Neighbourhood Night Service operates overnight across the whole of Leeds providing care for patients requiring nursing intervention in their own home. The service operates centrally from Killingbeck Court, Leeds LS14 6FD. One of the key aims of the service is to prevent unnecessary hospital admissions and to ensure that palliative patients remain in their own homes, where this is their preference. We are also involved in supporting the development of the Virtual Ward (Frailty) within the Neighbourhood teams. The Neighbourhood Night Service is part of a wider multi-disciplinary team and ensures that there is 24/7 nursing care for patients.
Main duties of the job
The community staff nurse plays a key role in supporting patients to remain in their own home overnight by providing quality care. The community staff nurse also provides support to families/carers overnight during different crisis situations i.e. blocked catheters and palliative care support.
You will be a registered staff nurse and have excellent assessment, planning and evaluation skills in order to ensure that patient care is timely and appropriate.
This is a fast paced role requiring safe and effective clinical decision making, so you must be able to think on your feet, problem solve issues that arise overnight and juggle conflicting priorities. You must be able to motivate others in challenging times and utilise the resources available to you effectively.

The post holder will be responsible for their designated workload within their speciality area with access to the support, advice and management from a senior clinician whilst not directly supervised. In doing so, they will assess, plan, deliver and evaluate patient centred programmes of care ensuring that they practice within their sphere of competence and knowledge. The post holder will work to professional protocols and regulatory body codes, standards and guidance at all times ensuring that their practice is grounded in the best available evidence based theoretical and practical knowledge.
