Job summary
There are three main functions of the Senior Nurse operating within the night service during the night;
- To clinically coordinate and manage care to ensure that all patient calls are effectively assessed, triaged, allocated and recorded. You will need to have an overview of the service as a whole to ensure that both patients and staff remain safe and care is appropriate and effective overnight. For example, reallocating work to cover unexpected staff absence. (The times for the Coordinator shifts are 20.45pm until 07.15am).
- To deliver clinical care to patients and support our Nursing Assistants overnight (The times for clinical shifts are 20:45pm-07:15am).
- Provide clinical leadership, including line management of band 5 Staff Nurses within your area of responsibility and oversee line management of our band 3 Nursing Assistants who are managed by the Staff Nurses.
Main duties of the job
The Senior Nurse plays a key role in ensuring that communication is effective to the staff on duty overnight and that any matters arising overnight are communicated effectively to relevant parties, which include the day function of the Night Service.
You will be a registered staff nurse and have excellent assessment, planning and evaluation skills in order to ensure that planned patient care is timely and appropriate.
You must have excellent communication, organisational and IT skills as you will be coordinating care for a large number of complex patients.
This is a fast paced role requiring safe and effective clinical decision making, so you must be able to think on your feet, problem solve issues that arise overnight and juggle conflicting priorities. You must be able to motivate others in challenging times and utilise the resources available to you effectively.

The post holder will clinically triage, prioritize, plan, allocate and manage all referrals that are made to the Neighbourhood Night Service including review of the caseload in conjunction with the night staff and Neighbourhood team. The role requires clinical decision making, reasoning and clinical triage. 1. Clinical
To utilize clinical knowledge to screen and triage all referrals received into the Neighbourhood Night Service.
To prioritize and agree a response time for all referrals received and accepted into the Team. This will be determined against the eligibility criteria, clinical need and capacity available within the team.
Be able to demonstrate sound clinical knowledge of the needs of people who have complex and multiple pathology diagnosis with a range of health and social care needs.
Be able to demonstrate an ability to collect information, problem solve and make decisions regarding clinical situations.
To carry out assessments of patient needs based upon clinical assessments undertaken by the clinicians
Ensure a full risk assessment has been completed as part of the triaging of referrals into the service.
Ensure patient records are maintained and accurate.
Maintain a detailed and up to date record of patients requiring planned intervention from the neighbourhood night service to facilitate service planning and allocation of resources.
To keep clinical skills up to date and relevant by regularly undertaking the role of the community staff nurse within the neighbourhood team
Engages and actively involves the individual and, if appropriate, their family/carer, in the assessment, planning, implementation of care
Works to standards of proficiency identified by their professional and regulatory bodies performing clinical treatments/procedures to a competent standard
Provides and receives complex, sensitive or contentious information where motivational, persuasive, empathetic, negotiating and reassurance skills are required whilst demonstrating an understanding of barriers to communication
