Job summary

We are looking for a dynamic and innovative cardiac rehabilitation physiotherapist to expand our Cardiac Activity Programme as part of the Community Cardiac Service in Leeds. You will have the opportunity to develop specialist skills in caring for cardiac patients and you will receive excellent mentorship and the opportunity to develop your clinical and leadership skills.

You will work as part of a multi-disciplinary team comprising of nurses, physiotherapists, occupational therapist, therapy assistant, and clinical support workers.

This is an exciting time to join the team as we expand the Cardiac Activity Programme, working alongside colleagues at Active Leeds to provide the best possible outcomes for cardiac patients. We are looking for an individual who embraces change, demonstrates strong resilience to the pressures within the NHS and continues to strive for excellence. A positive can-do attitude and the ability to work collaboratively with system partners is essential.

Main duties of the job

  • Work as an autonomous practitioner, managing a caseload of patients with complex cardiac conditions.
  • Support the provision and development of high-quality, safe, patient focussed cardiac rehabilitation services.
  • Provide cardiac rehabilitation in patients homes and remotely.
  • Work with Clinical Leads to improve clinical outcomes for patients.
  • Provide education and advice to patients / families to support healthy behaviour changes, lifestyle risk factor management and psychosocial health and wellbeing.
  • The ability to travel independently across the Leeds District is essential

In doing so, they will assess, plan, deliver and evaluate patient centred programmes of care to a specialist level ensuring that they practice within their sphere of competence and knowledge.
