Job summary
This exciting new role (fixed term 18 months) is to develop
a new service working between Leeds community Healthcare Neighbourhood Teams , Adult
Social Care and domiciliary care providers. This new initiative to provide
delegated nursing and social care tasks to a designated population. The post
holder will be responsible for the leadership and management of this service.
They will be pivotal in developing relationships across all parties and
providing the link between the neighbourhood team and the care providers. In
doing so, they will assess, plan, deliver and evaluate team performance and
delivery, ensuring that they practice within their sphere of clinical nursing competence
and knowledge. The post holder will work to organisational, professional, and
regulatory body codes, standards and guidance ensuring that their practice is based
upon evidence and practical knowledge. They will be responsible for developing this
new and innovative service responding to emerging knowledge and techniques.
They will deliver training, assess competency and build lasting relationships.
Main duties of the job
The post holder will maintain nursing clinical credibility
demonstrated through undertaking clinical practice and working alongside the
delegated care providers, delivering teaching, and assessing quality.
They must be able to practice as an autonomous nurse
to support service delivery, responding to social, scientific, clinical, and
ethical issues in line with the values and agreed model within the service.
They will be responsible ensuring the delivery of the clinical pathways
appropriate to the field of work. The role will include them to be responsible
for the performance and management of this new team. They will be expected to
delegate, organise and prioritise resources autonomously to ensure the safe delivery
of the service and making judgements in complex situations. They must be able
to work autonomously and able to develop relationships across areas. They will
be aligned into the Armley Neighbourhood team leadership structure but the work
will be based with care providers.
