An exciting opportunity has arisen for enthusiastic and dynamic Staff Nurses to join us at Leeds Community Healthcare NHS Trust and our Neighbourhood Teams in the West of the city delivering high-quality, patient-centred care in the community. Base locations & coverage areas are: Yeadon Health Centre Rawdon, Yeadon, Guiseley, Otley, Pool in Wharfedale Pudsey Health Centre Pudsey, Farsley, Calverley, Stanningley Holt Park Health Centre Horsforth, Tinshill, Cookridge, Bramhope Our community nursing teams deliver planned care in patients homes, helping people to remain independent and supported within their local communities. We provide a wide range of clinical interventions, including (but not limited to): Bowel and bladder management Wound care (including leg ulcer management) Pressure ulcer prevention and management Palliative and end-of-life care Use and management of enteral feeding tubes Safe administration of medication Through our work, we enable early discharge from hospital, reduce avoidable hospital admissions, and promote patient independence and wellbeing at home. The service runs 8am to 8pm, 7 days a week, with various shift patterns.

Main duties of the job

As a Staff Nurse in our Neighbourhood Teams, youll be part of a compassionate and highly skilled multidisciplinary team, working alongside District Nurses, Community Matrons, Occupational Therapists, Physiotherapists, Neighbourhood Clinical Assistants, Therapy Assistant Practitioners, and Self-Management Facilitators to deliver truly holistic, person-centred care. Youll also collaborate closely with our Citywide Services, including the Falls, Continence, Urology and Colorectal Care, and Palliative and End of Life Care teams, as well as GP services, ensuring the best possible coordination and continuity of care for patients across West Leeds. Key responsibilities include: Providing safe, evidence-based clinical care in patients homes. Maintaining excellent communication with patients, families, and colleagues. Delivering high standards of nursing care and supporting patients to achieve their personal health goals. Promoting self-management and independence wherever possible. Demonstrating resilience, adaptability, and motivation in a dynamic community setting. Upholding clinical governance standards and contributing to continuous service improvement.
