This is a great opportunity to join our Triage Hub based at Middleton Health Centre. We are looking for experienced and motivated senior clinicians with either a nursing or an AHP qualification who have a keen interest and/or experience in clinical triage.
You will be responsible for the triage and prioritisation of all new referrals as well as the prioritisation of planned and unplanned care for new & existing patients.
You will require expert clinical knowledge and judgement to aid your decision making. You will be expected to spend time in clinical practice to enhance and support this.
The role is an office-based role, but we can offer flexible working to support your work life balance.This service will operate from 7am-10pm, 7 days a week.
Main duties of the job
You will prioritise planned & unplanned care on an urgency basis along with supporting patients along their journey through services. You will work closely with primary and secondary care, as well as local third sector and voluntary organisations.
The Triage role supports patients to ensure they receive care in the most appropriate place, by the most appropriate team at the most appropriate time. You will be required to virtually consult with staff, patients, carers and other professionals.
You will work to professional and regulatory codes, standards and guidance at all times ensuring that your clinical practice is grounded in evidence based theoretical and practical knowledge. This is an exciting role in a dynamic service, and we welcome your application.
