Job summary
We're looking for an experienced practitioner with a strong sense of social justice to join our Homeless Health Inclusion Team (HHIT). We deliver transformational models of holistic healthcare to people experiencing homelessness, the Gypsy and Traveller community, and other inclusion groups in Leeds experiencing homelessness via outreach and in-reach programmes in partnership with other services. We advocate for our patients and plan safe discharge, providing out of hospital care and lead the health care for patients in Transitional Housing Units and those who have been placed in a health commissioned bed. We also provide outreach to the Gypsy and Traveller community, working with Leeds Gypsy and Traveller Exchange and facilitating health equity or their members. The HHIT team have links with the street teams, who provide street outreach, signposting and building relationships with those who are rough sleeping and those new to Leeds. The team works with those identified as suffering from homelessness and those identified as sofa surfing. People who are vulnerably housed, victims of trafficking, exploitation, vulnerable women fleeing domestic violence, refugees, those in the asylum process; failed asylum seekers and those with no recourse to public funding.
You will manage a team of healthcare professionals and care navigators and hold a caseload, working with patients on their health goals to ensure health needs are met and a reduction in emergency hospital admissions.
Main duties of the job
Duties:
- Responsible for delivery, leadership and management of the HHIT team.
- Developing new and innovative service developments.
- Ensuring the delivery of the clinical pathways.
- Support head of service to manage the service budget.
- Coordinating complex safe discharge planning from hospital.
- Identifying and implementing appropriate interventions.
- Providing education to stakeholders.
- Multi-agency working with all partners in Leeds who work with marginalised groups.
- Representing the service at city wide meetings.
The team is based at Burmantofts Health Centre but outreach and in-reach to various locations across the city. The service runs Monday to Friday 08:30 16:30.
You could be a Community Nurse, Health Visitor, Occupational Therapist or Mental Health Nurse. You will have had experience in working with people in inclusion health groups and working in challenging and complex environments. You're flexible and able to work under pressure and in a challenging and fast-paced environment. As you'll be working with people who have experienced complex trauma highly effective communication skills are a must. You are empowered by working independently but able to maintain a sense of team and your professional identity.
