Job summary

The Childrens Community Nursing Service has a part time vacancy for a clinical coordinator role to work in our newly established Clinical Co-ordination hub which operates 7 days a week, 8.30 5pm Monday to Friday and includes some weekend cover. They would work alongside others coordinators in the service and the management support officer (MSO) within the coordination hub. The aim of the coordination hub is to facilitate and coordinate clinical and operational care for children and young people accessing our service. The hub has oversight of the four clinical teams on a day-to-day basis and is the first point of contact for referrers, patients and clinicians. We are looking for a Band 6 nurse with expertise in various areas of community nursing being an experienced clinician who is motivated and keen to explore and test out different ideas and new ways of working to help improve how we coordinate services.

Main duties of the job

As this is a developing role post holders must be able to work flexibly and be proactive in developing and testing out new ways of working Responsibilities include; Coordinating clinical care and directing resources across the service daily Facilitating daily huddles to ensure that teams are safely staffed Supporting the management of staff sickness and reallocating work Triaging referrals and allocating these to ensure that children receive the right care at the right time by the right clinician Dealing with clinical queries from referrers, clinicians, and staff members and linking in with caseload holders as another layer of safety Appropriate skills and knowledge to cover a caseload as required. Robust knowledge and willingness to undertake difficult conversations. Updating care plans ensuring that key changes are well communicated Having oversight of clinical waiting lists and supporting teams to manage these Participating in and acting as a health expert on health panels, radar panels and other multiagency panels Providing clinical and child protection supervision to staff on both a planned and adhoc basis Maintain clinical competencies e.g. undertaking clinical work across all teams as the needs arise Co-ordinating and delivering clinical training sessions (both theory and practical sessions across all staff groups) A working knowledge of SystemOne and Health Roster or the willingness to learn these systems well is a crucial element of the role.
