In this role you will work in one of our 13 Neighbourhood Teams who operate from 07:00am - 22:00pm, covering a 7 day roster period, providing care for patients requiring intervention in their own homes.
The Neighbourhood Teams are part of a wider multi-disciplinary team to deliver 24/7 nursing care for patients in the community. This is an exciting role that incorporates many different aspects of patient care delivered to a wide range of patients.
We will also support you with a full tailored induction programme and will encourage and facilitate on-going development including the opportunity to develop and learn skills from team members from a variety of professions.
This vacancy is open to Newly Qualified and Experienced Registered Nurses.
Main duties of the job
As a Community Staff Nurse you will visit patients in their own homes undertaking a variety of clinical interventions. You must have excellent communication, organisational and IT skills. You will need to assess and respond to the changing needs of the patient, working within your own limits and seeking support from the wider clinical team where appropriate.
