Job summary

Integrated Neighbourhood Teams are in the early stages of development across Shropshire, Telford and Wrekin. Given Shropshire Community Health NHS Trust's unique position within the footprint, the Trust will be at the forefront of delivering this agenda. The introduction of this new role is designed to ensure that neighbourhood working is given the priority required to meet the needs of our population.

Shropshire, Telford & Wrekin (STW) is split into two Places - Shropshire (population of 330,000) and Telford & Wrekin (population of 190,000). Shropshire Place has been successful in the first wave of bids to the National Neighbourhood Healthcare Implementation Programme (NNHIP), this role will be key in supporting this work across the system.

The Trust also deliver 0-19 Public Health services in Dudley (population of 332,000) and whilst currently not playing a key part in the development of Integrated Neighbourhood Teams, there is a desire to be more fully involved.

Main duties of the job

  • As part of the Operations Directorate the postholder will plan and deliver neighbourhood development, identify and lead on associated growth opportunities and support allied productivity schemes in conjunction with service leads and corporate support services.
  • The role will include leading, planning and organisation of a broad range of complex projects and programmes, both new and ongoing, which require the formulation and adjustment of plans.
  • To work in partnership with senior leaders across the operations directorate and corporate services with key links to Planning and Quality divisions.
  • Develop productive working relationships with key external stakeholders including Primary Care Networks, individual GP Practices, local Acute and Mental Health trusts, STW ICB, both Local Authorities and local VCFSE (Voluntary, Community, Faith and Social Enterprise) organisations.
  • Work with operational, planning and procurement colleagues in tendering for services that are a strategic fit for the Trust and enhance our overall offer in STW, Dudley or beyond.
