Job summary

Shropshire Community Health NHS Trust have an exciting opportunity for a Band 5 community staff nurse to join the Northwest Community Nursing Team.

The successful applicant should be able to undertake a wide range of clinical skills and be willing to embark on further training as required. It is expected that the post holder will work closely with other members of the multidisciplinary team.

Previous community experience would be advantageous but is not essential. We also welcome applicants from newly qualified nurses and can offer an excellent preceptorship program and training for both the experienced nurse and newly qualified to gain the skills required. We are committed to clinical support, CPD & ongoing training.

  • 31.5 Hours per week

Main duties of the job

You will need to be qualified to RN level, be registered with the NMC and be able to demonstrate current clinical experience in a variety of NHS settings.

The service operates between the hours of 8am - 5pm and is a seven day, 365 days a year service. The community staff nurse role does involve working weekends and some Bank Holidays. As this is a community post the successful candidate will need to be able to meet the travel requirements of the posts.

Able to assess, plan, implement and evaluate interventions, liaising with colleagues and partners to ensure an integrated approach to the needs of the service user.

To organise and deliver skilled clinical support work without direct supervision. This will include individual in a variety of settings.

Gain valid informed consent and work within legal frameworks with patients.

To liaise with senior clinicians with regard to patient care, highlighting any variations to expected outcomes from agreed treatment programmes.
