An exciting opportunity has arisen for an experienced Band 6 Senior Nurse join our Integrated Community Rapid Response Teams.
The successful applicants will work within our Integrated Health and Social Care Team. This post will be working across the North East Shropshire localities (Whitchurch base).
The Rapid Response Service is a joint initiative between Shropshire Community Health Trust and Shropshire Council, working in partnership to provide our patients with access to rapid assessment, treatment and care to stabilise and support an acute episode of ill health or a deteriorating chronic condition that shows signs of heading towards a crisis. The service offers an appropriate alternative to being treated in an acute hospital environment delivering a recommended and agreed medical and nursing treatment / care plan. The focus is to avoid unnecessary hospital admissions and safely support people in their own home. The role and service offers integration and partnership working with team managers, nurses, social workers, care coordinators and support workers, across both organisations to ensure a seamless service.
The service operates 7 days a week, weekends and Bank Holidays, operating hours 8am-10pm.
Main duties of the job
We are looking for applicants that share our passion for advance nursing practice and quality of care, delivered to the person in their own home. This is an ideal opportunity for a Registered General Nurse to join the established nursing and care team and be part of shaping the service which is at the forefront of the NHS Long Term Plan agenda to develop urgent community response services. The service development links to the national reforms in community health services and aims to improve our local system responsiveness to managing community health crisis response services.
The successful applicants will be supported by the Team Leader and other Senior Nurses to provide assessment and delivery of comprehensive, holistic, skilled, evidence-based nursing and care support for the management of acutely unwell patients referred to the Rapid Response Team.

To be responsible for patient assessment and the planning, implementation and evaluation of individual care plans, in collaboration with the wider Multidisciplinary team together with patients informed consent, using nurse prescribing as appropriate to needs.
To provide immediate nursing care in the community in response to patient need.
Following a health assessment and clinical examination the Rapid Response senior nurse will use the non-medical prescribing qualification to prescribe timely medical treatments in order to prevent an admission to hospital.
To provide support to the emergency department to facilitate the safe discharge of patients to the community, to prevent admissions to hospital.
To respond appropriately to triage requests for assessments and care and to undertake triage in the absence of the team leader.
Train patients/carers/relatives (within the post holders locality) in necessary procedures to ensure a safe care environment for patients nursed in their own homes or within a care home environment.
