Job summary

We are recruiting two Band 6 Hospital at Home Practitioners based within the Harrow Integrated Care Team at Stanmore. This is an opportunity to join an innovative community service delivering acute, hospital-level care in patients' homes.

Central London Community Healthcare NHS Trust is rated Good by the Care Quality Commission and delivers high-quality, person-centred care across London and surrounding areas.

The Hospital at Home service supports adults with acute conditions through multidisciplinary care, including home-based monitoring, point-of-care testing, and intravenous therapy. Practitioners work autonomously while being fully supported by an experienced team.

Main duties of the job

The post holder will work autonomously and in collaboration with Acute Trusts, the Adult Community Nursing Team, other health care professionalsand statutory and voluntary agencies to deliver excellent standards of patient care.

The post holder willbe requiredto:

  • Work on a rotational shift basis within the Harrow H@H - service operating hours 8 am - 8 pm, 7 days a week.
  • Prioritise and manage own workload with minimal supervision.
