Job summary

We are recruiting x2 full time Band 6 Urgent Community Response Clinicians to join the Harrow Integrated Care Team, based in Stanmore, on a permanent basis.

Harrow Urgent Community Response is a nationally leading intermediate care service, delivering hospital level care in patients' homes to prevent unnecessary hospital attendance and admission. Working in pairs, you will respond to urgent referrals from GPs, the London Ambulance Service and Accident and Emergency departments, completing holistic assessments and initiating treatment within two hours of referral.

The role offers the opportunity to work autonomously in the community, with access to diagnostic equipment, hospital investigations when required, and real time patient records. We are looking for clinicians with a strong passion for patient care, excellent communication skills and a desire to develop beyond traditional boundaries of practice.

The service operates Monday to Sunday, 8:00am to 10:00pm.

Main duties of the job

As a Band 6 Practitioner, you will:

  • Attend patients as part of the Urgent Community Response team and independently for follow up visits, delivering holistic, patient centred care

  • Develop advanced clinical skills, including venepuncture, medicines administration, assessment of acutely unwell patients and acute injury assessment

  • Gain broad clinical experience across rehabilitation pathways, including generic, orthopaedic, musculoskeletal, respiratory, neurological care and falls prevention

  • Contribute to the day to day running of the service, including participating in handovers, supporting treatment planning during ward rounds and home visits

  • Support and supervise junior staff and students, contributing to their development and learning

  • Participate in in service training and access ongoing continuing professional development opportunities
