Central Londyn Community Healthcare are seeking a Bank Nurse Practitioner
The post holder will work autonomously and in collaboration with other health care professionals, statutory and voluntary agencies, and the Integrated Locality Teams (planned care) to deliver excellent standards of patient care for patients requiring support in unplanned care.
Key Working Relationships
To maintain, nurture and support working relationship with all staff working across the North Division, unplanned care team and across all integrated locality teams. Specific responsibilities in relation to locality managers/clinical lead, other integrated team colleagues of all professions and GPs.
Main duties of the job
- To provide highly skilled and autonomous care to a designated caseload including taking responsibility for the assessment, management, and evaluation of evidence-based nursing care to patients in a variety of community settings.
- Act as the Team Leader for the Rapid Response Nursing Team on a rotational basis responsible for day-to-day management of the service; acting as a key point of contact for GPs and other general practice staff.
- To work within and across professional and organisational boundaries, liaising with GP's other professionals and statutory and voluntary agencies to deliver individualised and expert nursing care to a high standard to client, and leading communication with all stakeholders.
- Take designated responsibility for discrete areas of operational management/ clinical practice.
- Work on a rotational shift basis - 8am - 4pm and 2pm - 10pm 7 day a week service.
- To support the Locality Manager/Clinical Lead and colleagues in delivering high quality care that delivers best value and meets the requirements of commissioners.
