Job summary
Job Purpose
The service delivers high quality care to patients at in their own home with a primary purpose to prevent hospital attendance/ admission and facilitate early discharge from hospital. The service operates from 8am to 10pm, 7 days a week.
The post holder will work autonomously and in collaboration with Acute Trusts, the Adult Community Nursing Teams, other health care professionals and statutory and voluntary agencies to deliver excellent standards of patient care.
The post holder will be required to:
Work on a rotational shift basis within the Herts Unplanned Care Team - service operating hours 8am 10pm 7 day a week.
Be the clinical leader for designated shifts on a rotational basis responsible for day-to-day management of the service.
Take designated responsibility for discrete areas of operational management, line management and clinical practice.
Prioritise and manage own workload with minimal supervision.
Main duties of the job
o Provide operational management and clinical leadership for the Unplanned Care Team including triage of referrals and ensuring efficient and effective use of resources via daily staff management of time keeping, work allocation and rest breaks, respond to immediate sickness absence (requesting cover as per protocol as required).
o As an Advanced Unplanned Care Practitioner act as role model supporting the team to achieve and maintain high standards of clinical care, based on evidence-based practice.
o As a senior team member and as an Advanced Practitioner the need to respond to clinical incidents, accidents and complaints in accordance with CLCH policy.
o Work in collaboration with and maintain a good working relationship within the other CLCH services such as adult community nursing, GP out of hours services, Accident and Emergency Departments, London Ambulance Service, acute hospital assessment units/ wards, General Practitioners and practice staff, nursing and care homes, Social Services and voluntary sector organisations so as to provide a comprehensive service to patients, clients and carers.
o Undertake assessment and appraisal of designated members of the team.
