We are hoping to recruit aBand 5 Discharge Co-ordinatorto join the CLCH inner London In-patient rehabilitation service across Athlone Rehabilitation Unit and Alexandra Rehabilitation Unit. We work as multidisciplinary team to ensure we deliver high-quality person-centred care in an environment that is friendly, patient and staff focused and rewarding.

You will be working with a varied multidisciplinary team consisting of; Occupational Therapists, Physiotherapists, Rehabilitation Nurses, GP's, Nursing Assistants, Rehab Assistants, SLT's, Dietitian's and Podiatrist's, to support the patients' experience and care and facilitate safe and effective discharges from the rehab units.

The successful candidate will have excellent communication and interpersonal skills and a passion for optimising the patients experience within the rehabilitation unit. We are looking for a dynamic, motivated, and enthusiastic member to join our friendly team.

Our bedded rehabilitation service aims to empower patients to recover from episodes of ill-health or injury and optimise their level of independence to enable them to enhance their quality of life and safely return home As a service we strive to provide a high standard of quality care and evidence based practice.

Main duties of the job

The successful post holder will:

  • Be involved in co-ordinating all stages of the patients journey from admission to discharge.
  • Proactively support and facilitate the work of the MDT in delivering the best outcome for the patient, informing, and communicating full outcomes of assessments and discharge plans with those involved in the patients care.
  • Support the Clinical Therapy Lead, Matron and Team Leads with implementing changes to improve patient experience and discharge experience.
  • Work closely with external agencies, discharge hubs and social services to ensure consistent flow is achieved across the units.
  • Follow up with transport and equipment providers to ensure the timely and smooth discharge of all patients
  • Produce reports and data collection to support the service in demonstrating safe and effective discharges and escalating concerns

Qualifications

Essential

  • NVQ level 3 qualification in health and social care or equivalent, or 2 or more A levels
  • Evidence of CPD.

Desirable

  • Training in discharge planning or case management.
  • Use of hospital IT systems e.g. System One
  • BSc, BA honours degree or equivalent
  • Professional registration as OT, PT, nurse or social worker

Experience

Essential

  • Ability to demonstrate a clear understanding of the role of a discharge co-ordinator.
  • Experience working with different healthcare professionals and understanding of the roles of the multi-disciplinary team (MDT) within a Rehab setting.
  • Ability to establish a good rapport and professional working relationships, in a clinical setting, with patients, family and friends, therapy, nursing and all members of the MDT who are involved in patient care.
  • Experience working autonomously and as part of a team including the ability to work under supervision and be aware of limitations seeking help as appropriate.
  • Experience of working in a team.
  • Experience of working with adults in health, social or educational setting either in paid or voluntary capacity.

  • Experience working in a Discharge Team or in a Discharge co-ordinator, progress chaser, pathway co-ordinator role
  • Knowledge of the Continuing Healthcare process and documentation
  • Experience in working with patients with physical disability or challenging behaviour within a rehabilitation setting.
  • Experience liaising with external agencies to support the service users needs such a social services, charitable organisations etc.

  • Good verbal and written communication skills, including the ability to keep legible and accurate patient records.
