An exciting opportunity has arisen for a Band 3 Rehabilitation Support Worker to join our Barnet Integrated Community Service by providing a high quality service to our housebound patients by facilitating earlier hospital discharges and preventing unnecessary hospital admissions.
Main duties of the job
As a Band 3 Rehabilitation Support Worker you would be responsible for providing safe, personalised and effective care to patients who require community therapy care by working alongside the community nursing team to deliver clinical and administrative duties.
Your main duties will include providing a number of clinical activities and holistic care under the direct delegation of a registered therapist using a wide range of clinical skills and interventions within a home environment.
You will need to possess excellent Communication skills and be a team player. You will also need to have good IT skills to support the systems in place.

- Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3)
- A recognised equivalent course
- Willingness to undertake further training as appropriate
Desirable
- GCSE grade A-C or equivalent in English and Maths
- ECDL certificate
- Registration with the national regulatory body (when it becomes available).

- Experience of working in a health or social care setting
- Experience of working as part of a team
