Job summary
Job Purpose
The post holder will support and deliver a high-quality clinical pharmacy service and medicines optimisation service to patients in Community Hospitals and ensure an integrated approach to the management of medicines and continuity of pharmaceutical care for patients admitted from secondary care/primary care into the Community Hospitals. The post holder may be in a rotational or fixed position as part of an integrated team and will support us in ensuring appropriate workforce across the sites.
The post holder will be required to work as an active member of the Medicines Management team to ensure the delivery of a safe, high quality and legal service which reflects national, local policy and priorities.
In this role you will be expected to travel and work across CLCH locations. Person needs to be flexible to travel to any of our Bedded Services sites within London (please see geographical CLCH map) A car would be useful for travelling but is not essential.
Main duties of the job
To provide and develop a high-quality specialist Clinical Pharmacy Service across a range of inpatient settings in the Community Hospitals (Rehabilitation Units, Interim Care Beds, Palliative Care, Discharge to Assess ward) covered by the Bedded Services team supported by the Lead Bedded Services Pharmacist.
To provide support and guidance to Medicine Management Pharmacy Technicians
To be responsible for individualised patient pharmaceutical care ensuring clinical prioritisation on a day to day basis at ward level.
To provide a patient focused pharmacy service to patients and staff includingmedicines reconciliation and full clinical screening of prescriptions to ensure safe and cost-effective use of evidence based and rational use of medicines, in line with local policies and national guidance ensuring medicines are optimised and risks to patients are minimized e.g. NICE, NHSE/I, PHE
