Job summary
Merton Domiciliary Therapies and Merton Holistic assessment and Rapid intervention Team is a Multidisciplinary Team providing high quality rehabilitation to Merton Population.
The post holder will support adults living with frailty and complex health needs to optimise their independence, health, and wellbeing, and prevent admission into hospital or premature move to a care home. We are strongly committed to the assessment and rehabilitation of patients who are at living within the Merton community.
The post holder has a key role within the team; working with patients, service users and their carers in providing highly specialist Physiotherapy (PT) assessment and treatment plans to assist individuals to maintain or regain their mobility, independence and wellbeing within their own homes, setting goals and reviewing outcomes in order to reduce the need for long-term support.
The post holder will provide local clinical leadership and advice for the Physiotherapist and MDT in domiciliary therapies and be responsible for the education and supervision of junior occupational therapy staff, rehabilitation assistants and PT students.

Main duties of the job
- Providing a highly specialist Physiotherapy (PT) service as part of a multi-disciplinary team, to patients in their own homes and within a clinic
- To provide clinically effective, person centred, and evidence-based Physiotherapy assessment and intervention to patients with complex physical health needs
- To assist the Clinical Lead in the planning and delivery of Physiotherapy services to the patients, their family/carers, students, Apprentices complying with Health and Care Professions Council(HCPC) and Chartered Society of Physiotherapy(CSP) professional and service standards of practice

- Educated to degree level with appropriate post graduate qualification in relevant clinical area
- HCPC registration
- Evidence Of CPD relevant to the treatment of people with physical disabilities
- Up to date CPD portfolio
Desirable
- Member of PT specialist interest group i.e. Specialist section in older adults.
- CSP registration
- MSC in appropriate clinical area.
Experience
Essential
- Post registration experience at band 6 level or above in a wide range of clinical areas i.e. elderly rehabilitation, community rehabilitation and orthopaedics, leading group classes etc.
- Experience of working in a falls and Rehabilitation team or a sound knowledge of falls and Rehabilitation
- Experience of working in MDT and leading Multidisciplinary meetings
- Postgraduate experience of working in a multi professional team
- Experience in teaching and training and facilitating Groups therapy sessions.
- Experience of assessing home environment and making onward referral to appropriate members of MDT within and across services
- Experience of completing Holistic assessments, interventions and care planning.
- Experience of completing triage
