Job summary

Central London Community Healthcare (CLCH) is one of the largest community healthcare organisations in London and Hertfordshire, providing our services to diverse communities/boroughs in 11 London Boroughs.

An exciting opportunity has arisen within our in-patient Intermediate care general rehabilitation service. We are looking for a band 3 Rehabilitation Assistant to join the therapy team at Midway Unit, Langley House.

This is a service for the rehabilitation of adults presenting with a range of physical dysfunction/or disabilities resulting from injury, illness, surgery or chronic disease. We are looking for a motivated individual to join the team, who has the passion and skills for supporting the therapy team in the delivery of a high standard of rehabilitation. You will be working with a multidisciplinary team that includes Physiotherapists, Occupational Therapists, Nurses, Dietitian, other Rehabilitation Assistants, Speech and Language therapists and Social Workers.

Main duties of the job

To be responsible for implementing prescribed treatment programs with an allocated patient caseload following successful completion of competency-based assessments and within agreed scope of practice. This would follow on from an initial assessment of each patient by a qualified member of the therapy team.

To work with all members of the multi-disciplinary team (MDT) in planning and facilitating the patient's progression through the treatment program, within the goals and protocols set out by the qualified staff.

To be able to drive a car to assist the therapist in completing access and home visits to patients property.

To assist therapists in completing paperwork, referrals and other appropriate forms that may be required during rehabilitation.

To take responsibility for delegated caseload once appropriate level of competency achieved and report outcomes appropriately, including reporting urgent and important information promptly to the appropriate person.

To review caseload on a regular basis with therapy team and report back appropriately when goals achieved/not achieved and if concerns are raised, or new information becomes available which impacts on intervention provided.

Essential

  • oHigher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3) or a recognised equivalent course
  • Willingness to undertake further training as appropriate

Desirable

  • GCSE grade A-C or equivalent in English and Maths
  • ECDL certificate
  • Registration with the national regulatory body (when it becomes available).

Experience
