CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

Due to maternity leave, we are looking for we are now welcoming applications from enthusiastic Physiotherapists with an interest and, ideally, experience in stroke rehabilitation to work collaboratively within the new Integrated Community Stroke Service (ICSS). Secondments would be considered.

This modern stroke service infrastructure follows the national service model and is delivered by a multidisciplinary team offering stroke/neuro rehabilitation at a range of intensities with patients in their own home.

If this is you, then come and work with us in our ICSS Team. We are seeking an enthusiastic skilled physiotherapist to be part of the ICSS team. You will form an essential part of a dedicated, knowledgeable multi-disciplinary team that regularly engages with team and individual clinical supervision. Development and training opportunities are available through the CLCH Academy.

Main duties of the job

To undertake comprehensive assessment, planning and treatment of patients with neurological impairment as a result of stroke using clinical reasoning methods to implement therapeutic intervention and provide patient centre intervention.

To write reports and contribute to multidisciplinary meetings to discuss patients.

To delegate, teach and supervise the work of rehabilitation assistants with guidance from senior staff.

To work with other health care professionals on joint assessments, patient goals and treatment programmes in a patient centred approach.
