Job summary

Job Purpose

To provide a high-quality specialist Clinical Pharmacy Service across a range of inpatient settings in the Community Hospitals (Rehabilitation Units, Interim Care Beds, Palliative Care, Discharge to Assess wards). To provide professional expertise, advice, support and training to health care professionals and patients within the Trust.

This role is required to cover Bedded Services in Hertfordshire and North London

Sites include:

- Langley House Hospital (698 St Albans Rd, Watford WD25 9FG)

- Hemel Hempstead Hospital (Hillfield Rd, Hemel Hempstead HP2 4AD)

- Potters Bar Hospital (Barnet Rd, Potters Bar EN6 2RY)

- Finchley Memorial Hospital (Granville Rd, London N12 0JE)

- Edgware Community Hospital (Burnt Oak Broadway, Edgware HA8 0AD)

Main duties of the job

To provide and develop a high-quality specialist Clinical Pharmacy Service across a range of inpatient settings in the Community Hospitals (Rehabilitation Units, Interim Care Beds, Palliative Care, Discharge to Assess ward) covered by the Bedded Services team supported by the Lead Pharmacist for Hertfordshire and Lead Pharmacist for Bedded Services.

To provide support and guidance to Medicine Management Pharmacy Technicians

To be responsible for individualised patient pharmaceutical care ensuring clinical prioritisation on a day-to-day basis at ward level.

To provide a patient focused pharmacy service to patients and staff including medicines reconciliation and full clinical screening of prescriptions to ensure safe and cost-effective use of evidence based and rational use of medicines, in line with local policies and national guidance ensuring medicines are optimised and risks to patients are minimized e.g. NICE, NHSE/I, PHE

Facilitate changes to the patients' own use of medicines to improve safety, adherence and empower as much independent use of medicines as possible and appropriate.

To participate in ward rounds and MDTs where appropriate to ensure a multidisciplinary and integrated approach to medicines management, with a view to ensuring continuity of pharmaceutical care for patients in different healthcare settings.

To optimise medications by undertaking Structured Medication Reviews and provide counselling to all patients

To participate in collecting and recording activity and intervention data to monitor key performance indicators.

To participate in ADR and Datix reporting and implement risk reduction initiatives at ward level

Education/Qualification

Essential

  • Vocational Masters Degree in Pharmacy.
  • Professional registration with the General Pharmaceutical Council (GPhC).
  • Postgraduate Clinical Pharmacy Certificate qualification or equivalent

Desirable

  • Postgraduate Clinical Pharmacy Diploma qualification or equivalent.
  • Independent prescriber.

Experience

Essential

  • Hospital experience
  • Experience of providing a Clinical Pharmacy Service with good medicines management initiatives
  • Evidence of basic teaching and tutoring in a hospital setting
  • Experience of working effectively within multi-disciplinary groups
  • Experience of structured medication review
