We are looking to recruit one dynamic and innovative pharmacist to join the Medicines Management Team for fixed term maternity cover, based in GP practices in Wandle Primary Care Network.

The postholder will gain experience in conducting structured medication reviews, carry out long term condition consultations, providing a high-quality clinical pharmacy service, working within a multidisciplinary setting with a variety of healthcare professionals and provide specialist expertise.

The postholder may also be required to travel to other sites and services within the organisation so will require flexibility as the organisation continues to grow and will include lone working. A car would be useful for travelling but is not essential.

Main duties of the job

The postholder must have post registration experience in either hospital or community pharmacy and/or GP practices. Previous experience in community health services or primary care is desirable.

A postgraduate qualification to at least diploma level or working towards or completed the CPPE Primary Care Pharmacy Education Pathway as well as independent prescribing qualification or currently working towards completion of this is desirable.

A sound knowledge of prescribing and medicines management issues faced in both primary and secondary care is essential with excellent decision-making and problem-solving skills.

Additionally, compassion, flexibility, excellent communication and time management skills are essential and the ability to build relationships with various health and social care professionals.
