Job summary

This is a fixed term position for a Clinical Pharmacist to work within the Wandle (PCN) in Wandsworth. The Pharmacist will work alongside GPs/ practice staff and other healthcare professionals to develop and manage medicines optimisation services within the PCN. They will perform face-to-face structured medication reviews as well as supporting GP practices within the PCN on ensuring robust medicines management processes are in place through clinical audit, review of medicines incidents, training and supporting clinical governance.

Main duties of the job

To provide clinical pharmacist support to Wandle PCN within Central London Community Healthcare NHS Trust (CLCH).

To provide pharmaceutical support to health care professionals (HCPs) and patients within the PCN.

To conduct face-to -face or remote structured medication reviews for patients in the clinic, practice setting, patient's homes or within care homes.

Support long term chronic disease management through polypharmacy review or specialist medicines optimisation prescribing clinics e.g. Diabetes/Asthma/COPD patients.

Utilise clinical tools to proactively identify patients for structured medication reviews.

Support QoF domains and improving disease registers.

To respond to patient medication queries regarding correct use of medications, side effects or other medication-related issues involving repeat prescriptions.

Education / Qualifications

Essential

  • Undergraduate Degree in Pharmacy
  • Professional registration with the General Pharmaceutical Council (GPhC)
  • Postgraduate clinical pharmacy qualification to at least diploma level
  • Independent prescribing qualification or working towards completion of prescribing qualifications within a specialist field e.g. diabetes, asthma, CHD

Knowledge

Essential

  • Up to date clinical and pharmaceutical knowledge
  • Sound knowledge of prescribing, pharmacy and medicines management issues faced in primary and secondary care
  • Understanding of the NHS and political environment

Skills

Essential

  • Excellent communication skills - both verbal and written including presentation skills and report writing
  • Ability to assimilate, analyse and interpret clinical and financial data.
  • Computer literate - working knowledge of office applications, internet, email and electronic diary systems
  • Ability to command and deal sensitively with emotive situations and be seen as credible by professional colleagues

Experience

  • Post-qualification experience of clinical pharmacy, medicines information and prescribing advice
  • Experience of working in secondary care and in a multidisciplinary environment
  • Primary care experience including Community Services
