We aim to empower patients to recover from episodes of ill-health and optimise their level of independence enable them to resume living at their usual residence.
This is a 24/7 service operating 365 days/year. The post holder will be expected to work shift patterns to cover the service including Days and Nights.
The Integrated Rehabilitation Support Worker will contribute to seamless, person centred, high quality, safe and clinically effective rehabilitation which is provided by the MDT.
The post holder will have an integrated role, working closely with Registered Therapists and Nurses and other team members in providing a flexible, supportive and responsive approach to meeting individuals rehabilitation and care needs.
They will communicate effectively with MDT to ensure that patient needs are met.
They will support a positive patient experience and rehabilitation by helping patients achieve their goals, outcomes, promoting an optimal level of independence and supporting family and carers.
They will provide compassionate care with courtesy, respect for patients and families, maintaining individuals dignity and promoting individual choice.
Main duties of the job
The post holder will be responsible for:
A caseload of patients working to agreed treatment objectives and to assist qualified staff in carrying out individual treatment programs.
Providing therapy and nursing interventions as part of a multidisciplinary team to patient within an agreed scope of practice
Successful completion of core competencies assessed by qualified staff.
Handing over patient progress and any areas of concern following intervention to the relevant therapist or Nurse.
Improving patient independence promotion of daily living skills, personal and social care.
