Job summary

Operating within the framework of the Hospital Discharge Services guidance (Aug2020), the post holder will work in partnership with colleagues in the acute sector, socialservices, continuing care and other community health services to ensure thatappropriate patients from the acute setting are assessed and transferred safely toappropriate community based services in a timely and seamless manner. Forappropriate pathways there is an aim to support transfer from hospital to communitywithin 2 hours or on the same day.To be responsible for ensuring smooth daily operations at the acute hospital bases andto act as a key point of contact and subject matter expert for community services tosupport with complex discharge queries. To lead daily huddles as required.

Main duties of the job

Make judgements involving highly complex facts requiring interpretation and analysis.

To prevent avoidable admission to acute care by undertaking autonomous, holistic clinical, functional and risk assessments and liaising with community practitioners and A&E staff to initiate the provision of appropriate services in the community

To prevent premature admission to long-term care by creative problem solving to find appropriate alternatives, using highly developed skills and knowledge of assessment andcommunication.

To support and enable the process of safe discharge from secondary to Community care and assist in bridging the gap from hospital to home.

To proactively promote a seamless and multidisciplinary approach to health and social careprovision.

Job responsibilities

  • To work as a core member of the acute based multi-agency Discharge Hub team to build and deliver the shared Vision for a Hospital Discharge Service. Where necessary, to act as a the CLCH on-site Team Leader and to be responsible for day to day management of the community health based elements of service delivery; including acting as a line manager for any other CLCH staff within the team.
  • To provide highly skilled and autonomous care to a designated caseload including taking responsibility for the assessment (if needed), management and liaison (external and internal) to patients in an acute setting.
  • To act as a pro-active community health service advocate, on behalf of CLCH and other Providers, through extensive knowledge of relevant community based pathways within the area of operation and ability to support effective discharge planning.
  • To act as a key point of contact for acute clinicians including Consultants, members of the Discharge Hub team and other community based clinicians/services.To work within and across professional and organisational boundaries, liaising withConsultants, ward managers, other professionals and statutory and voluntary agencies to deliver individualised and expert assessment and discharge guidance to a high standard, and leading communication with all stakeholders.
  • Take designated responsibility for discrete areas of operational management/ clinical practice.

Person Specification

Education/Qualification

Essential

  • Registered Nurse with current NMC registration or registered AHP with current HPC registration
  • Evidence of continuing personal and professional development with up to date CPD portfolio
  • Extensive post -registration qualification in advanced nursing practice, community nursing or equivalent relevant to the post

Desirable

  • Evidence of continuing professional education and training at Masters Level.
  • Management or Clinical leadership qualification.
  • Mentor or practice based learning training

Experience

Essential

  • Evidence of significant recent post-registration experience in community nursing, rapid response, emergency care, unscheduled care or equivalent therapy experience.
  • Experience of effectively working in collaboration, negotiating and liaising within a multi-disciplinary team and across organisational boundaries
  • Experience of urgent nursing/therapy assessments and developing and delivering care plans to meet these needs.

Desirable

  • Experience of conducting research projects and audit.
  • Experience of teaching others and mentoring students
