Job summary

o To plan, organize and evaluate an innovative health visiting service, and where appropriate,delegate to other members of the Health Visiting team to ensure delivery of a quality serviceo To deliver the healthy child programme and work in partnership with families to develop andagree tailored health plans to address their parenting and health needs.o To actively participate in service development activities.o To work in collaboration with other health care and service providers to ensure seamlessdelivery of services for clients and communities

Main duties of the job

  • providing health advice and health education programmes
  • undertaking developmental assessments of babies and children
  • helping people come to terms with issues such as postnatal depression
  • establishing and addressing key health needs
  • referral to and liaison with other relevant organisations
  • assessing parenting skills and children's home situations, and offering parents any further support that may be needed
  • working with other professionals such as social workers, GPs and school nurses

To provide a skilled, effective and evidence based health visiting service to clients in a varietyof community settings. To apply specialist skills and knowledge in the delivery of the health visiting service. To consult, advise and educate individuals and families on good health care practices. To develop health profiles of the defined population and local communities, identifyinghealth needs of disadvantaged groups and targeting outreach services to meet their needs.
