Job summary

We have an exciting opportunity for a Band 4 Senior Rehabilitation Assistant position within the Tri-borough Falls Prevention Service.

The service sees residents of Westminster, Kensington & Chelsea and Hammersmith & Fulham who have a history of falls, or who are at risk of falls. We predominantly see clients in clinics, deliver evidence-based strength and balance classes as well as in patient homes.

The post holder will work alongside a team of Physiotherapists, Occupational Therapists and other Rehabilitation Assistants and will be supported by their Team Lead and Falls Clinical Lead. The post holder will have a key role in helping to deliver strength and balance classes alongside therapists and will also work independently in clinic settings teaching exercises, checking memory, balance, fear of falling as well as measuring blood pressure.

As a Trusted Assessor, they will be supported to assess and prescribe appropriate equipment within an agreed framework to support clients in their own homes. The successful candidate will be given specific training on various outcome measures and screening tools and once competent will conduct these assessments independently. They will also be supported to provide basic falls assessments to returning referrals, and with PSI qualification will take additional responsibility when delivering group exercise classes.

Main duties of the job

- To be responsible for undertaking Basic Falls Screening for case and risk identification and other basic falls-related based assessments and interventions as advised by the Falls Specialist- To contribute to providing clients with the suitable and appropriate care in line with Falls Care Pathway and best practise guidelines- To be proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice- To be responsible for collecting a range of information on clients' needs and health status to assist the Falls Specialist in prioritising client care- To be responsible for supporting and/or independently supervising Falls Prevention Programmes with the Falls Specialist - To independently manage a caseload of non-complex cases in consultation with, and as delegated by Falls Specialist staff- To work in variety of settings including community centres, clinics, and clients' own homes- To assist in running the Falls Exercise and Activity Training (FEAT) group programme- To take on other duties such as clerical and administrative tasks as required- To provide cover to other Rehabilitation Assistants as required- To be prepared to train as a Postural Stability Instructor and to become a Trusted Assessor
