Job summary

o To plan, organize and evaluate an innovative health visiting service, and where appropriate,delegate to other members of the Health Visiting team to ensure delivery of a quality serviceo To deliver the healthy child programme and work in partnership with families to develop andagree tailored health plans to address their parenting and health needs.o To actively participate in service development activities.o To work in collaboration with other health care and service providers to ensure seamlessdelivery of services for clients and communities

Main duties of the job

  • providing health advice and health education programmes
  • undertaking developmental assessments of babies and children
  • helping people come to terms with issues such as postnatal depression
  • establishing and addressing key health needs
  • referral to and liaison with other relevant organisations
  • assessing parenting skills and children's home situations, and offering parents any further support that may be needed
  • working with other professionals such as social workers, GPs and school nurses

Leading and coordinating the healthy child programme by carrying out the 5 mandated contact visits.

Providing a seamless service to universal and vulnerable children and families by working in partnership with multidisciplinary teams.

Early identification, intervention, and appropriate onward referral. Good communication skills both written and verbal.

Reduced ability to deliver on KPI's

Patient safety will continue to be at risk

  • Evidence of continuing post registration education and training

Desirable

  • Mentor training or prepared to work towards

Experience

  • Recent health visiting experience
  • Be able to co-ordinate health promotion initiatives in line with local and national health improvement targets.
  • Experience of assessing, planning and implementing programmes of care and writing support to others
  • Experience in providing practice based learning support to others

Desirable

  • Office management and caseload
  • Critical appraisal skills and ability to utilise research evidence in practice
  • Ability to be flexible and responsive to client and organisational needs
