Job summary

Barnet Unplanned Integrated service works primarily as an admission avoidance service reducing the number of patients sent to hospital Emergency departments. We receive referrals from GP, LAS, 111, Residential and Nursing Homes to prevent admission of patients and support immediate discharge home from the emergency departments.

The Unplanned service also supports the early discharge home of patients on the D2A pathway 1 taking patients from the local acute hospitals.

We are a multi-disciplinary team consisting of Nurse Practitioners, Occupational Therapists, Physiotherapist, Rehabilitation Support Workers, and Technical Instructors with administrator support. This is an expanding admissions avoidance service working with both the primary care and the acute sectors to support and deliver treatment to patients in their own home. We are currently looking for a motivated and committed Band 6 nurses to join our team.

Main duties of the job

Candidates should be experienced nurse with sound clinical knowledge and proven experience in acute and community clinical environments. You will need to be adaptable to operate in a fast moving environment and have the clinical skills and ability to function as a lone worker in a larger team. Candidates should have excellent communication skills; and the ability to provide care as determined by the clinical assessments. Candidates will have undertaken professional development and have extended nursing skills, with evidence to support this. CLCH offers excellent training opportunities and we can support staff to complete the Non-Medical Prescribing course and Advanced Nurse practitioner course. The role requires travel around the local community you will need to have a full drivers licence and access to a car. The service covers 8am - 10pm working 7 days per week. The shift patterns are 8am - 4pm and 2pm - 10pm fulltime including weekend working
