Merton Community Neuro Rehab Team

(Fixed-Term Position - 12 Months Maternity Cover)

The Merton Neuro Rehab service is a multi-disciplinary service providing neurological rehabilitation to patients (aged 18+) with acquired neurological conditions. This includes an early supported discharge service for stroke patients.

Main duties of the job

Job description

Job responsibilities

  • To be responsible for the delivery of the Speech and Language Therapy service provided by Community Neuro Rehabilitation Team, providing assessment, implementing specialist SLT treatment and programmes of care, writing reports, reflecting specialist knowledge.
  • To provide specialist assessment, diagnosis and intervention for clients with complex acquired speech, language and swallowing difficulties, who require a domiciliary service. These adults will present with a wide range of cognitive, sensory, physical, behavioural, social, and communication difficulties.
  • To conduct interpretive, functional and instrumental assessment using specialist clinical/technical skills, demonstrating highly developed auditory, perceptual and physical skills in the assessment, diagnosis and treatment of clients. Furthermore, to use investigative skills and specialist knowledge of standardised assessments and other assessment tools, together with advanced analytical clinical reasoning skills in order to form a baseline measure or differential diagnosis from which to plan intervention.
  • To use a holistic, client centred approach, gathering and synthesising information from other professionals and relatives/carers, in order to ensure that intervention packages are best suited to patient, relatives and place of delivery.

Essential

  • Graduate Diploma / Degree in Speech and Language Therapy or equivalent
  • Registered with Health Professions Council
  • Member of the Royal College of Speech and Language Therapists
  • Evidence of postgraduate study or qualification in dysphagia

Desirable

  • Level 3 / 4 videofluoroscopy training

Experience

Essential

  • Broad postgraduate experience of working in the older people's rehab pathway including neuro rehab (acute, inpatient, and/or community)
  • Experience of working in a community setting
  • Experience of developing others through CPD / PDP / clinical supervision
  • Experience of working as part of a SLT team and/or multi-disciplinary team
  • Experience of service development

Desirable

  • Experience of working closely with other therapists within older people's rehabilitation
  • Experience of working within care home settings

  • In-depth knowledge of older people's rehabilitation including the impact of long-term conditions and neuro disorders e.g., Parkinson's Disease (PD), Motor Neurone Disease (MND), head injury, stroke, etc.
  • Competent in handling the mental, emotional, and psycho-social aspects of working in older people's rehabilitation
  • Familiar with relevant current legislation and clinical guidelines relating to community rehabilitation
  • An in-depth understanding of the nature of communication as a science including its function, processes, and pathology. This will enable effective communication, monitoring, analysis, and adaptation of interactions and responses within the assessment and therapeutic process
