Job summary

Rehabilitation Assistant - Band 4 -Maximising Independence Team - Wandsworth.

Could you deliver high quality care enabling people to increase their independence and allow them to remain at home where possible? We are seeking enthusiastic people with a positive attitude and commitment to providing excellent care and therapy support to our patients.

Maximising Independence is our community therapy service comprising occupational therapists, physiotherapists, speech and language therapists, support workers and administrators. It provides urgent and routine therapy to people over 16 years of age, helping them to regain their independence following illness or injury, and supporting them to manage long term health conditions.

We are looking for team players with a positive attitude and commitment to providing excellent holistic care. The post holder will provide support to patients in order to enable activities such as washing and dressing, preparing their meals, carrying out exercise programmes, walking, getting out into the community, access public transport etc. Care plans are prescribed by qualified therapists and you will be supported to deliver these programmes by the professional staff.

Main duties of the job

Your ongoing professional development is important and you will be supported to develop with opportunities to develop your skills in health care provision and rehabilitation alongside the therapy and nursing led services within the Borough. You will need to have strong communication skills, both verbal and written, and be able to use your initiative and judgement in order to evaluate, work with and feedback information to the therapy team on the abilities and needs of patients. You will need good IT skills in order to access and add to our electronic clinical record system. You will need to be able to operate as a lone worker out in the community with the ability to travel across the Borough in an efficient manner to patient visits ideally through access to your own transport. You would be supported with expenses for travel and parking.

Successful post holders must be able to work 37.5 hours across 5 days a week

  • A recognised equivalent course
  • Willingness to undertake further training as appropriate.

Desirable

  • GCSE grade A-C or equivalent in English and Maths
  • ECDL certificate
  • Registration with the national regulatory body (when it becomes available).
  • PSI qualification
  • Trusted assessor training
