Community Heart Failure Specialist Nurse - Band 7 Part time (18.75 hrs) - Wandsworth

An exciting opportunity for an experienced Community Heart Failure Specialist nurse to join our friendly team. The service covers Wandsworth providing complex care for people with Heart failure involving supporting secondary and primary care, Community integrated services and other healthcare professionals. Our specialist nurses are expected to be deliver complex Heart failure management together with providing education to patients to improve self-management of their condition.

You will be

  • A team player who's willing to work collaboratively with the multi-disciplinary team to deliver the best care possible for our patients
  • To have strong relationship building skills to work positively in your nursing team, multidisciplinary teams and with patients
  • Have a strong commitment to the delivery of high quality, safe, patient care and be a good role model in reflecting the Trusts core values
  • Have advanced clinical knowledge in the management of patients with Heart Failures
  • Be competent to offer guidance and supervision to members of the clinical team
  • Have excellent communication skills, comprehensive clinical assessment skills and the ability to lead on patient care provision to colleagues in primary and secondary care settings.
  • You will be able to adapt to a fast moving service and possess the clinical skills and ability to work independently and as part of a larger multi-disciplinary team

Job responsibilities

  • To use specialist Heart Failure knowledge and skills to safely identify, maintain and manage a caseload of heart failure patients, providing them with appropriate treatment, support and advice.
  • Monitor Heart Failure patients immediately following discharge in order to prevent readmission.
  • To contribute to the development of care pathways for people with Heart, ensuring appropriate NICE guidelines are implemented.
  • To undertake comprehensive assessments of complex Heart Failure patients with co- morbidities, including physical examination, and to develop treatment plans for patients using investigative and analytical skills, and to formulate individualised management and treatment plans using clinical reasoning.
  • To accurately undertake specialist clinical skills including administration of medication within National and local Guidance.
  • To interpret and analyse clinical and non-clinical tests to form an accurate assessment and suitability for the admission avoidance schemes, co-ordinate appropriate services to ensure safe transfer of care to the community and develop comprehensive discharge plans.

  • Post basic qualification in cardio-respiratory care or equivalent experience
  • Qualification in Independent nurse prescribing or willing to undertake training.

  • Has relevant degree or evidence of working towards degree

  • Extensive experience working in Heart Failure either in primary or secondary care
  • Highly Experienced in managing a clinical case load
  • Experience in the training of students
