Job summary
The Harrow Integrated Intermediate Care Coordination Service (ICCS) brings together NHS, Harrow Local Authority and Community and Voluntary sector services in Harrow to provide integrated hospital discharge, community rehabilitation and community reablement via a Single Point of Access.
We are looking for an enthusiastic leader who can engage with and support integrated post-discharge care co-ordination to ensure the best outcomes for patients across our discharge, rehabilitation and reablement pathway.
The Harrow Integrated Intermediate Care Coordination Service (ICCS) Manager will work closely with system partners associated with patient discharge, rehabilitation and reablement across Harrow, providing leadership to facilitate discharge and prevent re-admission to hospital. The post holder will also have accountability for the Discharge Hub Team based at Northwick Park Hospital.
Main duties of the job
If you can clearly demonstrate your managerial experiences to manage and deliver integrated services, an ability to work with multiple stakeholders and meet tight deadlines whilst being flexible within an evolving system then this a role for you!
This role is being recruited to as a 12-month fixed term contract and would also be considered as a secondment opportunity.

Job responsibilities
- Monitor, review, and report performance in relation to discharge, rehabilitation and reablement across all dimensions of care and delivery.
- To work as a system lead for the delivery of the organisational and system wide agreed discharge, rehabilitation and reablement standards and related policies.
- To take a lead in operationalising and implementing of any future guidance and legislation relating to discharge, rehabilitation and reablement across the system
- To act as a senior representative of their local system within the Northwest London Collaborative where appropriate to ensure strategic level input in relevant fora.
- The post holder will be required to support in the management of a range of system wide projects and initiatives developed to ensure the system meets its long, medium, and short-term objectives, in conjunction with NHS and non-NHS partners. While the majority of the role will operate in core working days and hours, it is also expected to support seven-day working and will be required to cover some evenings and weekends each month.
