Job summary
Community Heart Failure Nurse - Band 5 Full time (37.5hrs) - Wandsworth
We have an exciting opportunity for a new role as a Community Heart Failure nurse to join our friendly team. The service covers Wandsworth providing complex care for people with Heart failure involving supporting secondary and primary care, Community integrated services and other healthcare professionals. Our specialist nurses are expected to be deliver complex Heart Failure management together with providing education to patients to improve self-management of their condition.

You will be
- Team player who is willing to work collaboratively with the multi-disciplinary team to deliver the best care possible for our patients
- Strong relationship building skills to work positively in your nursing team, multidisciplinary teams and with patients
- Strong commitment to the delivery of high quality, safe, patient care and be a good role model in reflecting the Trusts core values
- Willingness to gain clinical knowledge in the management of patients with Heart Failure
- Excellent communication skills
- Able to adapt to a fast moving service and possess the clinical skills and ability to work independently and as part of a larger multi-disciplinary team
