Job summary

Specialist Community Public Health Nurse (Health Visitor) - Band 6 - Hammersmith and Fulham

Within Hammersmith and Fulham (H&F) CLCH provide a range of services including Health Visiting and School Nursing,

Children's Therapies as well as numerous adult services, this means that you will benefit from a close working relationship with peers and a joined-up approach to care leading to improved health outcomes for the people you care for.

Within your team you will benefit from a Borough lead with a wealth of experience in Children's health, Safeguarding, School Nursing and Immunisations and a team that has in excess of 300 years' experience who are ready to support your learning and development.

The team in Hammersmith are based within Parkview Centre for Health and Wellbeing, Richford Gate, Park Medical centre, Parsons Green Health Centre.

For registered Band 6 Health Visitors, this post is offering a premium of £2,500 payable upon commencement of employment (T&C's apply)

Main duties of the job

To plan, organize and evaluate an innovative health visiting service, and where appropriate, delegate to other members of the health visiting team to ensure delivery of a high-quality service

To provide a skilled, effective and evidence-based health visiting service to clients in a variety of community settings.

To work in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities

To collaborate with team colleagues in order to deliver the Quality Strategy objectives.

  • Utilising specialist skills and knowledge, advise and support individuals and families on evidence-based health care practices.

  • Initiate referrals to multi-disciplinary/agency colleagues as required to ensure adequate care and support for families and children.
