Job summary

We are looking to recruit a Band 7 DSN to join our dynamic team - however we would consider recruiting into Band 6 position with development into a Band 7 for the right candidate.

We are a multidisciplinary team consisting of Consultants, Dietitians, Podiatrist as well as Specialist Nurses. The team receives referrals from many primary care and community services as well as local acute hospitals.

We are a unique community provider as we manage people with both Type 1 and Type 2 diabetes and are an initiating centre for Freestyle Libre. We also deliver DAFNE & DESMOND structured education.

We are based in Morden (at the end of the Northern line) and work within the London Borough of Merton. Morden Underground station is just around the corner and Wimbledon Station 1.5 miles away. We are also on many bus routes, with a tram station very nearby.

Main duties of the job

  1. To work as a pro-active clinical nurse specialist to patients, carers and health professionals across the accepted domains of clinician, educator, service delivery and leadership across the Merton area.
  2. To work closely with the Service Lead to provide a Tier 3 Diabetes Specialist Nursing Service, that contributes to the strategic planning and delivery of the multidisciplinary Community Diabetes Service and is in line with the Trust objectives
  3. To be a key member of the multidisciplinary Community Diabetes Service to plan and deliver high quality integrated diabetes care in the community across Tiers 1 - 3
  4. To provide specialist assessment and treatment of patients with diabetes, liaise with other agencies and health professionals and maintain accurate clinical records
  5. To be responsible for project managing the planning, establishment and continued support of specialist nursing interventions

Job description

Job responsibilities

  • To be responsible for providing a specialist clinical diabetes service to clients with diabetes on oral hypoglycaemic agents, insulin or insulin conversion and other new therapies, including appropriate counselling and lifestyle advice in various community settings such as GP practices, health centers and clients homes
  • To work both autonomously and as part of a diabetes multi-disciplinary team (MDT) to provide expert advice to clients, carers and clinicians. This will involve assessment and the development of treatment plans using evidence based outcomes and involving patients throughout the process
  • To use professional judgement and clinical expertise to independently interpret and analyse complex situations and information to formulate treatment plans according to evidence based practice and modify according to changes in clinical condition
  • To be clinically competent to give specialist diabetes advice, treatment and counselling to support behavioural change, in adults with complex and diverse healthcare needs across health and social care. Resulting in personalized care planning, empowering service users to self manage their own diabetes
  • To provide a nurse prescribing facility and/or expert advice to medical practitioners of all oral hypoglycaemic agents used in Type 2 Diabetes, as well as Insulins and injectable therapies
  • To respond to urgent referrals and undertake a triage function in partnership with GPs and secondary care services
  • To initiate and participate in multi-disciplinary/multi-agency collaborative working to promote continuity of care at the multi-disciplinary Community Diabetes team meetings and case conferences
  • To use clinical judgment to refer clients to other services, professionals and Social Services when appropriate
  • To provide timely and informative client reports and discharge summaries for GPs, consultants, allied Health Professionals and other staff
  • To communicate highly complex and sensitive information in an understandable form to clients, carers and families from a wide range of backgrounds to gain consent, ensure understanding of condition, treatment options, risks, acceptance and compliance with dietary treatment, which may include unwelcome or difficult to accept information e.g. long term adherence to a medication, conversion to insulin
  • To use highly developed negotiation, persuasion, motivation and counselling skills to facilitate acceptance, behaviour change and ongoing compliance with treatment programmes taking into account emotional, physical, social and psychological barriers as well as strong influences on change e.g. depression, language difficulties, impaired cognitive function, resistant attitudes and religion
  • To be ultimately responsible for managing a specialist clinical caseload
  • To provide clinical expertise to GP Practices in their delivery of high quality diabetes care, in line with the Healthcare for London
  • To be a source of clinical expertise to GPs and other health professionals via a number of mechanisms including telephone and emails
  • To be professionally and legally accountable for all aspects of own work and to ensure that own work is within professional standards and clinical guidelines
