Central London Community Healthcare NHS Trust is large community trust working with its local PCNs looking to recruit a dynamic and innovative pharmacist to join the Medicines Management Team for an exciting post as a Band 7 Clinical Pharmacist.
Key components of the post include:
There is opportunity to develop in both primary and secondary care, with this rotational post. This is a permanent contract, working as part of the PCN providing specialist expertise in the use of medicines, in addition to working on Bedded Services, gaining experience of clinical pharmacy in the rehab setting. Responsibilities include conducting structured medication reviews for patients with inappropriate polypharmacy and overprescribing, developing and implementing long term condition prescribing clinics in primary care and acting as a source of pharmaceutical expertise to various health and social care professionals e.g. GPs, hospital clinicians, district nurses, carers, families, community pharmacists and care coordinators.
Main duties of the job
The Clinical Pharmacist will be supported by our PCN pharmacy team and Bedded services pharmacy team, having the opportunity to share good practice and develop the services.
The post will include working in Bedded Services in West London as well as GP practices within Hammersmith and Fulham North PCN and will involve lone-working and travelling between sites. The postholder may also be required to travel to other sites and services within the organisation so will require flexibility as the organisation continues to grow. A car would be useful for travelling in outer London or outside of London but is not essential.
The postholder must have significant post registration experience in hospital and/or community pharmacy. Previous experience in community health services or primary care is desirable. A sound knowledge of prescribing and medicines management issues faced in both primary and secondary care is essential with excellent decision-making and problem solving skills. Additionally, compassion, flexibility, excellent comunication and time management skills are essential and the ability to build relationships with various health and social care professionals e.g. GPs, hospital clinicians, district nurses, carers, family, community pharmacists and care coordinators.
