Job summary

We have an exciting opportunity for a diabetes educator to join our busy and dynamic Community Diabetes Team. This is a fixed term job, covering a secondment for 10months. You will be an enthusiastic and motivated individual, able to work as part of a multi-disciplinary team and support primary care health professionals to deliver structured diabetes education. You will be involved with supporting the development and delivery of structured education.

Main duties of the job

The services provide complex care to patients with diabetes within the community and deliver structured education to individuals with type 2 diabetes. You will be delivering diabetes education programmes which has been embraced by the community. In addition, you will be supporting with all administrative work related to the delivery of the structured education. We work very closely with our G.P Practices to support them to manage diabetes care within the community, part of your role will be communicating with the GP Practices.

The post-holders will have excellent communication and organisational skills. We actively encourage innovation and are committed to professional development.

  • To follow the prescribed training programme for DESMOND Educators (non-HCP), which will include:
  • Observing a local DESMOND education course. Attending 3 separate days of training organised by the DESMOND National Programme which may involve travel in order to attend
  • Committing to a mentorship programme delivered locally by an assigned mentor (an experienced and accredited healthcare professional DESMOND Educator)
  • Using the development workbook and other resources to support ongoing development as a DESMOND Educator
  • To participate in the DESMOND Training and Quality Development Framework.
