Job summary

Health Care Assistant - Band 3 - Wandsworth

We are looking for exceptional Band 3 Health Care Assistant to join us in our Central London Community Nursing Day Service, the service is run from 07.00am to 17.30pm.

The team cover The borough of Wandsworth, crossing North, East, South and West Localities.

You should hold NVQ level 3 in health and social care and have a minimum of 12 months acute or community experience. If you are a motivated team player and want to excel in delivering exceptional holistic healthcare to a diverse patient group in their own home, we want to hear from you.Previous experience of working in the NHS is essential, community experience is desirable but training will be given. As a Health Care assistant, you will be directly involved in facilitating and shaping self-care for patients with long term conditions in order to help keep them as well as possible in their own homes. Main duties of the job

The Health Care Assistant will provide a mixture of general and specific care to a group of patients under the guidance and supervision of a registered nurse or allied health professional. The HCA will work in a variety of settings including community centres, clinics and patient / carers own homes.

The post holder will be expected to become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice. The HCA will promote optimum independence and individualised client centred care to a delegated caseload.

The post holder will also support the Community Nursing Team to deliver against key performance indicators and maintain quality of the service.

Job description

  • To contribute to the on-going assessment of a patient by completing the appropriate paperwork for allocated patients, recording and reporting all clinical and behavioural information to the nurse/therapist for action and planning purposes.
  • To undertake, and report back on delegated clinical activities that have previously been signed competent. E.g.: vital signs, routine urinalysis, glucose monitoring, insulin administration, mobility training, falls prevention awareness groups, increasing independence through activities of daily living
  • To enable patients/carers to make most effective use of resources within the service by ensuring they receive up-to-date information sheets, responding to any questions constructively
  • To pass onto/seek assistance from other colleagues, promptly, any questions that you are unable to resolve
  • To liaise and refer patients to the MDT team as appropriate in accordance with local procedures.

Essential

  • Willinqness to undertake further training as appropriate
  • GCSE grade A-C or equivalent in English and Maths

Desirable

  • ECOL certificate
  • Registration with the national regulatory body (when it becomes available).
  • Experience of working in a health or social care setting
