Job summary

Hospice Therapy Assistant - Band 4 - St Charles Centre for Health and Wellbeing

An opportunity has arisen for a keen and proactive person to join the multidisciplinary palliative care team based at Pembridge Hospice. We support people living with a life-limiting illness to live life to the fullest.

The role will involve supporting and assisting the occupational therapist and physiotherapist to deliver rehabilitative palliative care to patients, face-to-face and online, in outpatient appointments, groupwork and home visits. Duties include clinical and administrative tasks. A full training and induction programme will be given to ensure the post holder achieves the required level of skill.

We are based at St Charles Centre for Health and Wellbeing, a short walk from Ladbroke Grove station and we see patients in Brent, Kensington and Chelsea and Hammersmith and Fulham.

You must have strong communication skills and be able to work autonomously to prioritise their responsibilities. You should have knowledge of the roles of occupational therapists and physiotherapists working in palliative care.

Previous experience working as part of a therapy or palliative care team would be advantageous.

Main duties of the job

The palliative rehabilitation assistant is an active member of a team providing high quality, evidence-based rehabilitation for patients with life-limiting illnesses in community, day hospice and inpatient settings.

The rehabilitation assistant will take referrals from the occupational therapist and physiotherapist. They will work both independently and with supervision to deliver individualised therapeutic treatment, working towards patient-centred goals. They will hold and plan their own caseload together with advice and support from therapy staff.

They will see patients as lone workers, monitor the patient's condition and progress and report back to the occupational therapist and physiotherapist.

Palliative patients often have complex and fluctuating physical, social, emotional and psychological needs because of their health condition and varying treatment including surgery, radiotherapy and chemotherapy.

  • Devise and implement a recommended individual therapy programme, approved by the qualified therapists, or implement a therapy programme devised by the qualified therapists. These programmes will include activities such as exercises, safe mobility practice, personal care, non-pharmacological symptom management, and feeding. Interventions may be carried out in individual and group settings as appropriate, in outpatients, day hospice or in patients homes. Interventions will be carried out both independently and jointly with the qualified therapists.
  • Plan and organise groups for patients, including relaxation, exercise and breathlessness.
  • Demonstrate activities to volunteers and instruct volunteers in how to assist groups.
  • With direction / guidance from the physiotherapist and / or occupational therapist, use evidence-based principles / current best practise to assess, plan, implement and evaluate interventions.
  • Work towards rehabilitation goals set by the patient in collaboration with the team and carer as appropriate, appreciating the physical, psychological, social, spiritual, and cultural needs of each patient.
