Job summary

CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

West Hertfordshire community respiratory service provided by CLCH is looking for an enthusiastic forward thinking respiratory nurse specialist to join our friendly expanding multi-disciplinary team.

The post holder will provide patient centered integrated care, providing specialist services including hospital at home (admission avoidance scheme), Virtual Hospital, home oxygen assessment & review service and nurse led clinics and home visits to provide expert specialist care to patients with COPD, asthma, bronchiectasis, OSA and ILD. Working effectively within a multi-disciplinary team and demonstrating effective communication skills is essential.

Main duties of the job

To work within a multidisciplinary respiratory team to deliver community respiratory service in West Hertfordshire providing specialist services including hospital at home (admission avoidance and early supported discharge scheme), home oxygen assessment & review service and nurse led clinics and home visits to provide expert specialist care to patients with COPD, asthma, bronchiectasis, OSA and ILD.
