We are a dedicated 14 bedded rehabilitation unit consisting of a multi-disciplinary team which includes Registered nurses, Occupational Therapists, Physiotherapists, GP, Pharmacist, Rehabilitation Support Workers and Health Care Assistants. As a team, we provide high quality, comprehensive patient-centered rehabilitation with the aim to maximise goal attainment, patient independence and well-being.
This role provides an exciting opportunity to work a passionate and dynamic multi-disciplinary team where new ideas are encouraged to continually improve processes and patient care.
This is a full-time position of 37.5 hours per week

Main duties of the job
This post is within a new purpose built rehabilitation unit, which is located in a care home in Merton. The unit offers rehabilitation to patients to improve their physical function. A specialist multi-disciplinary team (MDT) optimises the patient's recovery following the rehabilitation pathway.
Most patients are admitted to the unit after an acute admission to hospital for approximately 3 weeks rehabilitation prior to discharge back to their home environment.

- To undertake specialist and holistic assessment of patients as an autonomous practitioner, interpret the results to form accurate diagnoses and prognoses; use clinical reasoning skills and knowledge of evidence based practice, in relation to the individuals clinical and psycho-social issues to determine treatment options, recommending the best course of action.
- To undertake treatment planning and implementation of agreed goal orientated intervention with client/carers.
- To contribute towards single assessment and trusted assessment processes
- To provide comprehensive intervention to patients in the community, making use of assessment and treatment facilities where appropriate.
- To be responsible for a clinical caseload working within multi-disciplinary teams, sharing care appropriately ensuring clients and carers receive optimum level of therapy and holistic care within available resources.
- To carry out multi-disciplinary screening assessments and discuss outcomes with multidisciplinary team members.
- To keep accurate up-to-date records on patient care and equipment provision, which comply with quality standards of Trust/Professional/Service policies, meet legal requirements and that can be shared with colleagues and carers when appropriate.
- To provide complex written and/or verbal reports to referrers and other agencies involved in clients care and participate in clinical meetings as necessary.
